Shockwave in physiotherapy

Shockwave in physiotherapy

A strong addition to your clinical toolbox

Shockwave therapy has gained significant ground in modern physiotherapy in recent years. It is evidence-based, minimally invasive, and can be used either as a supplement to or an alternative for traditional methods such as manual therapy, exercise, and electrotherapy. Shockwave delivers fast and measurable results, and many clinics report increased patient satisfaction and growing demand.

Common shockwave indications in physiotherapy

Shockwave therapy is used daily in physiotherapy clinics to treat conditions such as:

  • Plantar fasciitis and heel spurs
  • Achilles tendinitis and tendinopathy
  • Tennis elbow and golfer’s elbow
  • Calcific shoulder tendinitis (tendinitis calcarea)
  • Trochanteric bursitis and gluteal tendinopathy
  • Myofascial trigger points
  • Fascial treatment and scar tissue

With both focused (fESWT) and radial (rESWT) shockwave therapy, you can tailor the treatment to the depth of the tissue and the nature of the pain. When shockwave is combined with exercise and other physiotherapeutic interventions, the results reach an entirely new level of effectiveness.

How shockwave works in practice

A typical shockwave session lasts 10–20 minutes and can be easily integrated into a standard consultation. Many patients experience noticeable improvement after just 1–3 sessions, with most treatment plans consisting of 5–6 sessions. Shockwave stimulates the body’s own healing processes and enhances the effectiveness of other interventions you already offer in your practice.

The treatment is gentle and free from side effects. It requires no anesthesia and can be combined with exercise therapy immediately afterward. This provides a key advantage: your patients often experience reduced pain and increased mobility right away—resulting in better movement quality during exercise and faster clinical outcomes. A benefit to both patient and therapist.

Clinical documentation

Shockwave therapy is supported by numerous randomized controlled trials and meta-analyses. The International Society for Medical Shockwave Treatment (ISMST) has published a cross-manufacturer guideline that summarizes the most important studies on shockwave therapy across a wide range of clinical indications.
You can access the guideline here.

In addition, shockwave therapy is endorsed by NICE (UK) for the treatment of plantar fasciitis and calcific tendinopathy.

Studies show significant improvements in both function and pain reduction compared to placebo treatments or injections.

Benefits of using shockwave in your physiotherapy practice

  • High patient satisfaction and clinically documented results
  • Attractive as a private-pay service – typically EUR 45–65 per session
  • Strengthens your differentiation in a competitive market
  • Increases patient loyalty through longer treatment plans
  • Can be used at the first point of contact – no referral needed
  • Suitable as a stand-alone service or as a complementary modality

Financial viability

The investment in shockwave equipment can quickly pay for itself—often within just a few months. With an average session price of EUR 55–70 and a typical treatment course of 4–6 sessions, you can easily generate EUR 270–400 in revenue per patient. It's entirely realistic to increase your clinic’s monthly earnings by up to EUR 3,350.

Even with just 3–5 patients per week, a leasing agreement or upfront investment is often fully covered within the first six months. Many clinics also report a noticeable increase in patient flow through word-of-mouth and targeted marketing efforts.
